Progressive multifocal leukoencephalopathy with selective involvement of the pyramidal tracts.

A 67-year-old man with chronic lymphatic leukemia presented with a 1-month history of subacute right hemiparesis with right central facial palsy and right extensor plantar response, but without sensory deficits. MRI showed hyperintense lesions along the pyramidal tracts on T2-weighted and FLAIR images (figure). CSF PCR revealed JC virus infection leading to the diagnosis multifocal leukoencepha...

Unusual MR Feature Presenting Discrete Involvement of Pyramidal Tract in Progressive Multifocal Leukoencephalopathy: A Case Report

Progressive multifocal leukoencephalopathy (PML) is a demyelinating disease caused by reactivation of JC virus in immunocompromised patients. To date, PML with discrete involvement of the pyramidal tract has been described in only two patients. This report describes an additional case with PML showing discrete involvement of the pyramidal tract on T2-weighted images and FLAIR images.

Progressive Multifocal Leukoencephalopathy

Before embarking on experimental therapies for progressive multifocal leukoencephalopathy (PML), the diagnosis needs to be unequivocally established. Improving the underlying immunodeficiency state is the best initial approach to the management of PML. Immunosuppressive therapies should be discontinued when feasible. In the patient with AIDS, highly active antiretroviral therapy should be admin...

Progressive Multifocal Leukoencephalopathy.

Progressive multifocal leukoencephalopathy is a subacute demyelinating disease that occurs in patients with defects in cell-mediated immunity, including those with AIDS and lymphoproliferative disorders. It is caused by reactivation of JC virus (JCV), which infects 70% to 90% of the population by adulthood, but remains latent in normal hosts. Once reactivated, JCV infects oligodendrocytes and a...
